Who signs off what, on which evidence, and what a conditional sign-off means. Each sign-off is a named person confirming that a defined set of checks has passed on an identified baseline, or that any exception has been waived by someone with the authority to waive it. Sign-offs roll up into the gate decision; they do not replace it.
The owner assembles evidence on a fixed baseline, the signer checks it, exceptions go for waiver, and the sign-off is recorded with any conditions.
