1. Purpose of the gate
Review and sign-off policy approved makes sure that accountability for every deliverable stays with a named engineer once an AI-assisted step is in the flow. The tool never signs. The risk this gate addresses is subtle: an existing sign-off, such as a design review or a verification sign-off, can come to rely on work that nobody actually reviewed, simply because the work arrived looking complete.
The gate criteria are a human review policy approved, with review depth set per failure mode; an accountability matrix with a named sign-off owner for every affected deliverable; review checklists and sign-off criteria updated; and disclosure guidance agreed with legal. The marker Review policy frozen is reached here: from now on any change to review depth goes through quality, and lighter review needs evidence from monitoring.
This gate freezes more decisions than any other in the early lifecycle, because review depth depends on what was learned in qualification, which projects may use the step depends on the data and terms review, and disclosure depends on the licence terms. Reviewers should expect to see those earlier records cited, not restated.
