Coverage exclusion
A recorded decision to remove a coverage item from the denominator because it is unreachable or out of scope, for example logic disabled in the current configuration.
Exclusions must be reviewed and signed off, since they directly inflate the reported score.

Notes
Exclusion files are version-controlled artefacts; an unreviewed exclusion is the easiest way to hide a real bug.
